One person killed, four people severely injured near 421 N PULASKI RD in Humboldt Park

A driver was killed; and three passengers and another driver were severely injured after a Saturday night head on collision in Humboldt Park on June 8, 2019 at 9:22 PM.

The crash occurred near 421 N PULASKI RD, on an undivided road. The posted speed limit was 30 mph. It was dark but with streetlights, the weather was clear, and the roadway was dry. The police report did not specify a cause for the crash.

There were two motor vehicles involved in the crash:

• A 2012 Kia Optima was heading south. The driver, a 17-year-old boy, was not wearing a seat belt and his airbag deployed; he was killed. A 23-year-old male passenger was not wearing a seat belt and his airbag deployed; he reportedly suffered incapacitating injuries.
• A 2007 Honda CR-V was heading north. The driver, a 37-year-old woman, was wearing a seat belt and her airbag deployed; she reportedly suffered incapacitating injuries. A 16-year-old passenger was wearing a seat belt and their airbag deployed; they reportedly suffered incapacitating injuries. An 18-year-old female passenger was wearing a seat belt and her airbag deployed; she reportedly suffered incapacitating injuries.

Police were notified at 9:22 PM and made a report on scene. This was the 379th injurious crash reported in June 2019, and the 6,490th injurious crash reported in 2019. It was the 40th fatal crash for 2019.
